Hospice Care

Eligibility
  • Individuals with one of more serious illness who would benefit from pain and symptom management.
Services Provided
Services Provided
  • Hospice serves any patient who is clinically eligible for the benefit
  • Eligibility is defined as a life expectancy of six months or less if the illness runs its normal course.

Palliative Care

Eligibility
  • Medically necessary Physician and Nurse Practitioner visits
  • Expert pain and symptom management
  • Registered Nurse and Certified Nursing Assistant visits as needed based on plan of care
  • Physician and/or Nurse Practitioner visits
  • Expert pain and symptom management
  • Spiritual and social support
  • Volunteer services as desired
  • Caregiver respite when needed
